Reverse T3 is one of the most misunderstood thyroid markers — yet it explains why so many people have thyroid symptoms despite “normal” labs. Reverse T3 is a molecule the body creates when it wants to slow down metabolism as a protective response. The more Reverse T3 your body produces, the less active thyroid hormone is available for your cells.

Reverse T3 is made from T4, the same hormone used to create active T3. But instead of increasing metabolism, Reverse T3 blocks thyroid receptors, preventing T3 from doing its job. It is not inherently bad — but when levels become too high, it creates a physiological “brake” that slows energy, mood, digestion, and overall function.

The most common triggers that raise Reverse T3 include:

  • Chronic stress or elevated cortisol
  • Inflammation or illness
  • Gut dysfunction or nutrient malabsorption
  • Low-calorie diets or fasting
  • Over-exercising or poor recovery
  • Blood sugar instability

These patterns signal the body to conserve energy rather than expend it.

Reverse T3 becomes especially important when someone feels clearly hypothyroid — fatigue, coldness, weight gain, brain fog — yet their TSH and T4 appear normal. In these cases, the issue often isn’t thyroid hormone production, but thyroid hormone utilization.

High Reverse T3 doesn’t mean the thyroid is weak — it means the body is trying to protect itself.

Stress is one of the biggest drivers. When cortisol rises, the body prioritizes survival over metabolism. It diverts T4 into Reverse T3, slowing cellular function. This is why thyroid symptoms often worsen during stressful periods even without changes in labs.

Inflammation creates the same effect. When the immune system is activated — whether from infection, dysbiosis, food sensitivities, or chronic gut irritation — the body lowers metabolic rate to conserve resources. This shift increases Reverse T3 and decreases active T3 at the tissue level.

Low caloric intake is another major trigger. When someone under-eats — intentionally or unintentionally — the body perceives scarcity and increases Reverse T3. This is why dieting often makes thyroid symptoms worse, not better.

Blood sugar swings also impact Reverse T3. Fluctuations force cortisol to intervene, and cortisol shifts conversion toward Reverse T3, explaining why people with unstable glucose frequently feel sluggish or cold.

Addressing Reverse T3 requires understanding why the body is producing it. Reducing stress load, supporting gut health, improving nutrient status, and stabilizing blood sugar often lower Reverse T3 naturally. When Reverse T3 decreases, thyroid symptoms typically improve rapidly.
