It’s one of the most confusing experiences for people with Hashimoto’s: you start thyroid medication, your TSH improves, your T4 improves… but your antibodies don’t budge. Sometimes they even rise. Many people assume this means their medication isn’t working, but the real explanation is far simpler: medication supports thyroid hormones — not the immune system.  You can see how we assess this distinction on the Thyroid Page.

Hashimoto’s is an autoimmune condition, which means the immune system — not the thyroid — is the source of the antibodies. Medication replaces or supports thyroid hormone levels, but it does not address the immune triggers that cause antibodies to remain elevated.

The most common reasons antibodies stay high include:

  • Gut inflammation or intestinal permeability
  • Chronic stress or dysregulated cortisol
  • Hormonal fluctuations that amplify immune activity
  • Nutrient deficiencies that impair thyroid repair
  • Viral, environmental, or food-related immune triggers

These patterns keep the immune system activated, even if thyroid hormone levels appear stable on paper.

When thyroid labs improve but antibodies stay elevated, it often means the immune system is still responding to something upstream. For example, gut irritation is one of the strongest drivers of autoimmune activity. When the gut lining becomes inflamed or permeable, the immune system becomes more reactive, making it far more likely for antibodies to remain high. Stress is another powerful factor. High or inconsistent cortisol increases inflammatory signaling and suppresses the enzymes that help regulate thyroid hormone activity. Many patients notice their antibodies rise during stressful months, even when nothing has changed with their medication.

Medication improves hormone numbers — antibodies improve when immune triggers calm down.

Hormonal changes also play a major role. PMS, ovulation, postpartum shifts, and perimenopause can all temporarily increase antibodies. This is why some people see month-to-month antibody fluctuations that make little sense until the hormonal connection is recognized.

Another overlooked reason antibodies stay high is ongoing inflammation, sometimes from sources people don’t immediately connect to the thyroid — blood sugar swings, chronic under-eating, certain medications, environmental exposures, or nutrient depletion. Selenium, zinc, vitamin D, iron, and omega-3 fatty acids are essential for immune regulation, and deficiencies make antibody reduction much slower.

It’s also important to remember that antibodies do not always drop immediately. For some people, stabilizing antibodies takes months — not because the condition is worsening, but because autoimmune patterns shift gradually. What matters most is reducing the triggers that keep the immune system agitated.

Stable thyroid labs are not the same as a stable immune system. Medication helps one part of the picture. Antibodies reflect another part entirely — one that requires looking beyond replacement hormones and evaluating what’s influencing immune activation.
