One of the most common digestive misunderstandings is believing that burning, reflux, or upper abdominal discomfort means you have too much stomach acid. In reality, most people have too little. Low stomach acid creates symptoms that mimic high acid — which is why so many people are incorrectly treated with acid-suppressing medications. Low stomach acid causes food to sit in the stomach longer than it should. When meals don’t break down efficiently, the stomach fills with pressure and partially digested food. That pressure pushes acid upward into the esophagus, creating burning or discomfort that feels identical to high acid. A major cause of low stomach acid is H. pylori. This organism disrupts acid production, slows digestion, and delays gastric emptying. When acid is low, food ferments in the stomach, increasing gas and pressure — which then drives reflux. Many people with reflux don’t have excess acid; they have excess pressure. T

Low stomach acid also contributes to dysbiosis and yeast overgrowth, especially Candida. Acid normally acts as a barrier that prevents microbes from reaching the intestines alive. When acid is low, more organisms survive and colonize the gut, contributing to bloating, fermentation, and stool inconsistency.  Stomach acid also signals the pancreas to release pancreatic enzymes. When acid is insufficient, enzyme release becomes weak. This leads to undigested food entering the small intestine, feeding dysbiosis and worsening downstream symptoms. 

Low acid further disrupts bile flow. Acid entering the small intestine triggers the gallbladder to contract. When that signal is weak, bile becomes sluggish, contributing to fat malabsorption and elevated steatocrit. This produces heaviness after meals, floating stools, and nausea.

Another reason low acid is misdiagnosed as high acid is mucosal hypersensitivity. When the gut lining is inflamed — reflected by elevated secretory IgA or calprotectin — even small amounts of acid feel irritating. This inflammation-driven sensitivity connects with and helps explain why acid suppression offers temporary relief but worsens symptoms long-term.

Low acid also contributes to intestinal permeability. When proteins aren't broken down fully, they irritate the gut lining and trigger zonulin, weakening the barrier. This leads to food reactions, inflammation, and fatigue — symptoms often confused with “sensitivity to acid.”
