Facial puffiness, swelling under the eyes, fluid retention in the hands or abdomen, and that “inflamed, puffy” evening feeling are often blamed on food reactions, hormones, or salt. But one of the most common root causes is dysbiosis — an overgrowth of opportunistic bacteria that produce inflammatory metabolites and disrupt the body’s fluid-regulation systems. The GI-MAP identifies these patterns through dysbiosis markers, yeast clues, inflammation indicators, bile flow issues, and permeability signatures.

The first way opportunistic bacteria cause puffiness is through metabolite accumulation. Bacteria like Klebsiella, Citrobacter, and Morganella produce ammonia, alcohol-like compounds, sulfides, and other metabolites that irritate blood vessels and tissues. These compounds accumulate throughout the day, creating the classic “fine in the morning, puffy by evening”.

Another major factor is inflammation-driven fluid shifts. Dysbiotic organisms elevate secretory IgA or intestinal inflammation, which increases vascular permeability — allowing fluid to move into tissues more easily. This produces puffiness around the eyes, cheeks, lower face, and abdomen. 

Yeast overgrowth]] adds another layer. Candida produces acetaldehyde and other byproducts that directly affect the lymphatic system and fluid balance. This contributes to facial swelling, under-eye puffiness, and a “bloated face” appearance, especially later in the day. 

Dysbiosis also causes puffiness through poor bile flow. Bile helps clear inflammatory compounds and supports lymphatic movement. When bile is sluggish (often confirmed by elevated steatocrit), inflammatory metabolites accumulate, and the lymphatic system slows — creating swelling and fluid retention. 

Another contributor is intestinal permeability. When zonulin rises, the gut lining becomes “leakier,” allowing inflammatory particles to enter circulation and trigger body-wide swelling. Evening meals tend to worsen puffiness because permeability and inflammation usually peak later in the day. 

Opportunistic bacteria also influence blood sugar stability, contributing to swelling. When dysbiosis causes glucose dips and spikes, insulin fluctuates — and insulin is a fluid-retention hormone. This creates puffiness, especially around the face and midsection, and overlaps with other carb-driven patterns.

Another overlooked mechanism is histamine amplification. Dysbiotic bacteria and yeast can increase histamine load or reduce histamine breakdown. When histamine rises, swelling and facial redness worsen — particularly after meals. This reaction is often mistaken for food sensitivities when the real issue is microbial imbalance.

Finally, dysbiosis disrupts lymphatic flow. Healthy flora produce short-chain fatty acids that support lymphatic drainage. When beneficial bacteria are low and opportunistic organisms dominate, lymph movement slows — leading to puffiness, heaviness, and the feeling of fluid stagnation.

Facial puffiness and fluid retention are not random cosmetic issues — they are often microbial signatures, reflecting dysbiosis, inflammation, permeability, bile sluggishness, or yeast activity.  The GI-MAP reveals exactly which physiologic patterns are driving the swelling so treatment can address the true cause.
