Not all supplements support the gut — and for people with intestinal permeability (leaky gut), certain supplements can actually increase irritation and symptoms. When the gut lining is already inflamed or reactive, adding the wrong ingredients can raise zonulin, stimulate the immune system, or worsen fermentation. Understanding which supplements help versus harm makes a major difference in recovery.

One of the biggest issues is high-dose probiotics used too early. While probiotics can be beneficial, introducing them into a gut with active inflammation or dysbiosis can cause bloating, pressure, or immune activation. This is especially true when the microbiome is imbalanced or when organisms like Klebsiella, Citrobacter, or Candida are elevated. 

Another culprit is supplements containing fermentable fibers, such as inulin or FOS. These prebiotics are helpful when the gut is stable, but in the presence of permeability, they can feed the wrong organisms and increase gas, distention, or inflammatory reactivity. 

Digestive enzymes, although generally beneficial, can also irritate the lining when used improperly. If low pancreatic enzyme output is present, enzymes help — but taking them without food, or in high doses, can cause abdominal discomfort or burning sensations in people with an inflamed mucosal layer. 

Even herbal antimicrobials can worsen leaky gut when taken too soon. When the gut barrier is compromised, aggressive antimicrobials may cause die-off reactions, immune activation, or worsening of permeability. This is why the GI-MAP is essential — it reveals whether inflammation needs to be calmed before introducing stronger interventions. 

Supplements aren’t inherently bad — they just need to match the state of the gut.
